WAIVER AND RELEASE FROM LIABILITY
Description of Activity: All Activities
I hereby acknowledge that I am participating in DIY Studio 323 activities, located at 14013 Boyette Rd Riverview, FL 33569.
Acknowledgment of Risks:
I understand that participating in DIY Studio 323 activities may involve any risks, and take full responsibility, I acknowledge that I am voluntarily participating in these activities with full knowledge of the risks involved, and I agree to assume all risks associated with my participation, I allow DIY Studio 323 to use any photos taken during the activities on social media.
Release of Liability:
In consideration for being permitted to participate in DIY Studio 323 activities, I hereby release, waive, discharge, and covenant not to sue DIY Studio 323, its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me, whether caused by the negligence of the releasees or otherwise, while participating in DIY Studio 323 activities.
Indemnification:
I agree to indemnify and hold harmless DIY Studio 323, its owners, employees, agents, and representatives from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur arising out of or related to my participation in DIY Studio 323 activities.
Medical Authorization:
I authorize DIY Studio 323, its owners, employees, agents, and representatives to seek medical treatment for me in the event of an emergency and agree to be responsible for all medical expenses incurred.
Agreement to Terms:
I have read this Liability Waiver Form and fully understand its contents. I am aware that by submitting this form I accept the terms and conditions of this document, I am waiving All legal rights that I may have against DIY Studio
323 and its representatives.
